WORKSHEET INSTRUCTIONS:
STEP 1: Please enter your contact information.

STEP 2: Please enter the information that you know about the NSMN member that you want to meet. If you only know the company name but not the person's name, please enter it. If you don't know the person's name, but you know the job title you need, please enter "head of sponsorship," for example.

STEP 3: To make the Member-to-Member Introduction, NSMN will write a 2 paragraph introduction email between you and the other NSMN Member. Please write your paragraph here exactly how you want us to use it. For example: " Billy Smith is the Vice President of Smith Sports. He has worked in sports marketing for 15 years and ......"
